I got mine out today, fair warning attempt at own risk. I was able to do this without damaging the threads. If you do damage threads just order new adapter for 15 bucks.

Take flat head screw driver that is thick enough to fit into adapter but get stuck if you push it in, then hammer it into place, turn counter clockwise and out pops adapter. If you wrap flat head with tape or cloth you may be able to pull it out with out too much damage to inner threads. Try adapter over shifter, if tou can turn smooth then your good. Again worse case scenario you buy new adapter. But you have full function of shift knob again.
